Consent Forms for Health Services
One of the biggest obstacles to providing health services for children is receiving parental consent. A parental consent form must be signed for a child to receive health-related services.
With the support and help of our partner agencies, Stakeholders has compiled many of the consent forms needed for children to receive services from state and local agencies, mobile care providers, and in school-based health centers.

It is our hope that providing all of these forms in one place will help increase the number of students, especially those that are uninsured and medically underserved, that return signed consents and are able to receive health services.
