Complete the online form to schedule programs for your school. Or, download the program request form and return the completed form to Sara Turk, Project Coordinator, by email or fax (312) 293-3954.
We will contact the community partner that provides the program to confirm availability for your requested date(s) and time(s).
We will contact you to confirm your program request. If the program is not available for the date and time you requested, we will ask you to provide an alternate date(s) and time.
Our community partner agency will come to your school to deliver the program. If you need to reschedule or make changes to your requested date(s) and time(s) for programs, please contact us as soon as possible.
